- Temperature will be taken as part of patient intake. If you have a fever or are currently ill, please call to reschedule your appointment and stay home and rest.
- Face mask is recommended in the exam room due to proximity of care, but not required.
- COMPLETE our on-line welcome form PRIOR to your office visit. This is an important step to ensure we are prepared for you.
- Know your medical history and list of current medications
- Know your current symptoms and be able to describe them
- Know your family history
- Some eye diseases like glaucoma and macular degeneration are hereditary
- Bring your medical insurance card, identification and method of payment
- Bring your most recent prescription for glasses or contact lenses
- Bring your corrective eyewear to the exam
- If undergoing a test using dilation eye drops, bring proper eye protection, like sunglasses, for after your appointment.
